Consumer Rights: Access to Health Records & Complaints
In accordance with Texas Health & Safety Code §181.105, the following information explains how clients may access their health records, contact the licensing authority, and file a consumer complaint.
Requesting Your Health Care Records
Clients have the right to request access to their mental health records.
How to Request Records:
• Submit a written request via email or through the client portal.
• Requests must include:
o Client full name
o Date of birth
o Description of records requested
o Preferred delivery method (secure portal, encrypted email, or mail)
o Signature and date

Timeframe for Records Requests:
• Requests are processed within 15 business days of receipt.
• If additional time is needed, you will be notified in writing.
• Fees, if applicable, will comply with Texas law and will be communicated in advance.

Filing a Consumer Complaint
Clients may file a complaint regarding mental health services, privacy concerns, or professional conduct.
How to File a Complaint:
• Visit: https://www.bhec.texas.gov/discipline-and-complaints
• Select: “File a Complaint”
• Complete the online complaint form with supporting details and documentation
Complaints are reviewed by the appropriate licensing authority in accordance with Texas law. This notice is being posted in compliance with Texas House Bill 4224 (Effective September 1, 2025).
